If you previously received dental care at Gentle Dental of Brandon and would like to return to Parsons Family Dental, you may need to request that your dental records be sent to our office.

Can I request a copy of my dental records?
Patients generally have rights to access copies of their health information. HHS states that individuals have a right to see and obtain copies of their health information, subject to limited exceptions.

Can my dental records be sent to Parsons Family Dental?
A patient can direct a covered entity to transmit PHI to another designated person or entity through a qualifying written and signed request that clearly identifies the recipient and where the information should be sent.

How long does a dental records request take?
Processing times may vary depending on the office maintaining your records and the nature of your request.
We recommend submitting your request before your scheduled appointment when possible.
Will Parsons Family Dental request my records for me?
Our team can provide information to help you understand the records-transfer process. Depending on the circumstances, you may be asked to complete and sign an authorization before records can be released.
